Momentum Medicine Plus, LLC, established in 2013, has grown in providing multiple government and private medical services. MMP prides itself in offering objective, reliable, and prompt medical expertise. MMP is an established and highly reputable medical provider in the field of IME’s (Independent Medical Evaluations), CE’s (Consultative Examinations), and C&P (Compensation & Pension) examinations.

Dr. Juan Cornejo

Physician

Dr. Cornejo grew up in the Washington DC region. He earned his medical degree from Kirksville College of Osteopathic Medicine in Missouri. He later moved to the Philadelphia area where he did his internship and residency. After completing a sports medicine fellowship, he worked as a nonsurgical medical orthopedic and sports medicine physician at University Orthopedic Institute at Hahnemann Hospital in Philadelphia for several years. He is currently board certified in Family Medicine and Primary Care Sports Medicine. He has treated orthopedic conditions including neck, back, shoulder, elbow, hand, hip, knee, ankle, and feet pain. He was also a clinical instructor at Drexel Medical School. He has taught a number of students and residents during his years in practice.

After working many years in academic and private practice, Dr. Cornejo decided to open up his medical practice in NJ. He started Momentum Medicine Plus, LLC in New jersey with his first office in Voorhees, NJ. His practice started out performing social security disability examinations for the States of New Jersey and Virginia. Eventually the practice grew to over 8 offices throughout the state of NJ and 2 offices in Virginia. Doctor Cornejo’s training in orthopedic medicine and work injuries as well as training by the ABIME as a certified independent medical examiner, allowed him to offer objective medical expertise for disabled patients. He prides himself in offering an objective medical examinations and understands the importance of these exams for patients who are suffering a hardship from their disability.

As evident by the humanitarian award he received in medical school, he believes in helping those in need and in pain. He has traveled to Mexico three times providing prolotherapy for those who are suffering from chronic pain conditions and who may have had little access to medical care. Likewise, he treats each and every one of his patients like a human being rather than a number. He prides himself in being caring and understanding.
He currently lives in South Jersey with his wife. He has 2 boys who enjoy soccer, piano, and swimming. He has a baby on his way as well. He has coached his older son soccer since his son was 6 and is now doing the same for the younger one.

I am a competent professional who can develop and maintain working relationships with physicians, staff and patients. I possess strong Human Resource Management skills, Administrative skills and Data Management skills.

Heather digristina

Practice Manager

Mrs. Digristina is the practice manager at Momentum Medicine Plus. From 1989 to 1991 Heather served as the recording secretary for FBLA (Future Business Leaders of America) at her high school and participated in the early work study program. She then turned her interest to the medical field as she trained at Star Technical Institute as a Medical Assistant. She currently has 26 years of experience and service in the medical field. Starting out as a medical assistant in 1996 and quickly taking on the role as practice manager to a busy multi-physician medical practice. She has been with Momentum Medicine Plus for the past 7 years assisting in the growth and development of the practice.
